Medicare Supplement Rates by State for 2027
Medicare Supplement Insurance premiums can vary substantially by company, plan, ZIP code, age, tobacco use, household discount, and the way the policy is rated. A national price range cannot predict what a particular 65-year-old will pay in 2027. For a reliable comparison, use Medicare’s ZIP-code Medigap search and obtain an official quote from each insurer.
In most states, policies with the same plan letter provide the same standardized benefits, but premiums can differ. Massachusetts, Minnesota, and Wisconsin use different state-specific designs. The sections below explain what current data can support, how state pricing rules differ, and what to verify before buying.

The Most Reliable National Benchmarks
The newest nationwide annual data are historical benchmarks, not 2027 quotes. KFF’s October 2026 Medigap analysis reports that the average monthly premium among current Plan G policyholders was $194 in 2025. State averages ranged from $164 in New Mexico to $257 in New York. Those averages include people of different ages and health profiles, so they should not be presented as the price for a new 65-year-old buyer.
KFF also found that average Plan G premiums increased 13% from 2024 to 2025. That is a measured historical change, not a forecast that every carrier or policyholder will receive a 10% to 20% increase in 2027. Rate changes depend on the carrier, policy block, state approval process, and other factors.
For high-deductible Plan G, the official 2027 deductible is $3,050. This deductible is separate from the monthly premium and does not establish what an insurer may charge.
What Determines a 2027 Medigap Premium
- Plan design: Compare the same lettered plan, or the same state-specific design in Massachusetts, Minnesota, or Wisconsin.
- Location: Premiums can vary by state, ZIP code, or rating area.
- Pricing method: Community-rated, issue-age-rated, and attained-age-rated policies can behave differently over time.
- Applicant details: Age, tobacco use, sex where permitted, and eligibility for a guaranteed-issue right can affect the quote.
- Discounts and payment method: Household and electronic-payment discounts vary by insurer. See the Medigap household discounts guide for questions to ask.
State Pricing Rules at a Glance
According to KFF’s 2026 review of state rules, nine states require community rating for policies sold to people age 65 and older. Four states allow issue-age rating but prohibit attained-age rating. The other 37 states and the District of Columbia permit insurers to use any of the three federal rating approaches. The carrier’s actual filing still determines which method applies to a specific policy.
| State rule | Where it applies |
|---|---|
| Community rating required for age 65+ | Arkansas, Connecticut, Idaho, Maine, Massachusetts, Minnesota, New York, Vermont, and Washington |
| Issue-age allowed; attained-age prohibited | Arizona, Florida, Georgia, and Missouri |
| Any federal rating method permitted | The other 37 states and the District of Columbia |
| Different plan standardization | Massachusetts, Minnesota, and Wisconsin |
State by State 2027 Rate Checklist
The table identifies the pricing framework to ask about in every state and DC. It intentionally does not insert a single premium range because an accurate 2027 quote requires a ZIP code and applicant details. Use Medicare’s cost-comparison guidance and confirm the final price with the insurer.

Special Plan Designs in Three States
Massachusetts uses Core, Supplement 1, and Supplement 1A. Medicare’s Massachusetts comparison explains the benefit differences. Supplement 1 includes Part B deductible coverage and is generally limited to people eligible for Medicare before January 1, 2020.
Minnesota uses Basic and Extended Basic policies and also offers Minnesota versions of Plans K, L, M, and N. This means it is inaccurate to say Plan N does not apply in Minnesota. Review Medicare’s Minnesota benefit chart before comparing prices.
Wisconsin uses one standardized base policy with optional riders. It also has 50% and 25% cost-sharing options and a high-deductible option. See Medicare’s Wisconsin benefit chart for the current structure.
Enrollment Rights Can Matter as Much as the Rate
Federal law provides a one-time six-month Medigap Open Enrollment Period that begins when a person is both 65 or older and enrolled in Medicare Part B. Certain losses of other coverage and certain Medicare Advantage changes can also create guaranteed-issue rights. Outside a protected period, an insurer may use medical underwriting unless state law provides additional protection.
Connecticut, Maine, Massachusetts, and New York have broader continuous or annual protections. Delaware and Rhode Island have annual protections for certain people moving from Medicare Advantage to Medigap, and Minnesota added an annual opportunity for certain people ages 65 to 70 effective in 2026. Rules differ, so verify eligibility with the state insurance department before dropping existing coverage.
If a Medicare Advantage plan leaves Medicare or stops serving an area, affected members may receive a protected window to buy certain Medigap policies. Medicare generally describes an application period beginning 60 days before coverage ends and ending 63 days afterward, but the exact right depends on the event and prior coverage. Use Medicare’s Medigap eligibility tool and keep the plan’s termination notice.
How to Compare 2027 Rates
- Choose the benefit design first. Use Medicare’s standardized benefit chart or the state-specific charts for Massachusetts, Minnesota, and Wisconsin.
- Search by ZIP code. Medicare’s Medigap policy finder displays estimated costs and companies selling policies locally.
- Request official quotes from multiple insurers for the same plan and the same effective date.
- Ask how the policy is rated, whether the quoted price includes discounts, and how often rates have changed for that policy block.
- Check whether switching requires medical underwriting. Free, objective counseling is available through SHIP, and state insurance departments can provide complaint and rate-filing information.
What Not to Use as a 2027 Quote
Do not treat 2020 city examples, statewide averages for existing policyholders, or Medicare Advantage plan counts as a 2027 Medigap quote. Medicare Advantage and Medigap are different markets. CMS publishes a separate 2027 Medicare Advantage and Part D state landscape, but those plan counts do not establish Medigap availability or premiums.
